SNF Nurse Practitioner, Full-time

Prima CARE is a 180-provider, physician-owned multispecialty group, based in Fall River,Massachusetts. We are seeking a Nurse Practitioner to join our team providing care to patients
in local skilled nursing facilities (SNF).

Prima CARE participates in managed care and full-risk insurance products, so efficient and cost-effective care is important to us. Prima CARE has adopted a coordinated model of care that covers our patients from hospital to SNF and then back home. Our goals are to maintain excellent care while in nursing facilities and avoid hospital readmissions.

When making rounds in the nursing facilities in the Fall River area, the Nurse Practitioner would work alongside other NPs and each facility’s medical director, determine management plans, and direct the care of SNF patients.

The Nurse Practitioner must be licensed in the Commonwealth of Massachusetts and must hold a valid Drug Enforcement Agency certificate and a Massachusetts controlled substances registration. The successful candidate must demonstrate maturity, organizational skills,independence, flexibility, and excellent problem-solving abilities. He/she must be able to work well with nurses, therapists, pharmacists, administrators, case managers, and physician colleagues. Compassion and willingness to engage patients and families in healing environment are key. Fluency in Portuguese is a “plus” but not a requirement.
