In this blog, we’ll walk you through five common signs of hemorrhoids and help you understand when it’s time to seek expert care.

1. Rectal Bleeding

One of the most common symptoms of hemorrhoids is painless rectal bleeding, especially during or after bowel movements. You may notice bright red blood on toilet paper, in the toilet bowl, or streaking your stool. This bleeding is usually linked to internal hemorrhoids, which form inside the rectum.

However, rectal bleeding can also indicate more serious conditions such as colorectal cancer or anal fissures. If you’re experiencing blood in your stool, it’s essential to consult a healthcare provider for a thorough evaluation and accurate diagnosis.

2. Anal Itching and Irritation

Persistent anal itching—also called pruritus ani—is often caused by irritation from external hemorrhoids. Swollen blood vessels near the anal opening can leak mucus or small amounts of stool, leading to itching or burning around the anus.

Itching may worsen after long periods of sitting or poor hygiene. Gentle cleaning, use of hypoallergenic toilet paper, and topical hemorrhoid creams may provide relief, but if symptoms persist, medical evaluation is advised.

3. Swelling or Lumps Around the Anus

A noticeable swelling or lump around your anus may indicate an external hemorrhoid. These lumps can be tender, especially when sitting or wiping. In some cases, you may develop a thrombosed hemorrhoid, which occurs when a blood clot forms inside the hemorrhoid tissue. This can cause sudden, severe pain and requires medical attention.

Skin tags may also develop after hemorrhoid flare-ups and are often mistaken for active hemorrhoids.

4. Pain or Discomfort During Bowel Movements

Hemorrhoids can make bowel movements uncomfortable or painful. Straining due to chronic constipation increases pressure on rectal veins, worsening internal and external hemorrhoids. You may feel a sense of incomplete evacuation, pressure in the lower rectum, or discomfort that lingers after using the bathroom.

A warm sitz bath, fiber supplements, and stool softeners can help relieve these symptoms. But if pain is severe or does not improve, further care may be necessary.

5. A Feeling of Fullness or a Prolapsed Hemorrhoid

In more advanced cases, internal hemorrhoids may prolapse, meaning they bulge out through the anus. This can create a sensation of fullness or a visible bulge during or after bowel movements. Prolapsed internal hemorrhoids may retract on their own or require gentle manual pushing.

If the prolapse persists or becomes painful, this may signal a more serious issue, such as a strangulated hemorrhoid, and should be evaluated by a rectal surgeon.

While mild hemorrhoid symptoms often respond to self-care measures, certain warning signs call for prompt medical evaluation:

  • Bleeding that lasts more than a few days
  • Sudden or intense rectal pain
  • Presence of a hard lump around the anus
  • Persistent itching or swelling
